• Certificate of : Employment, Career, Retirement, Receipt for Wage & Salary Income Taxes, and Withholding Tax

• A) Yourself : Bring your official ID card.
• B) Proxy : Bring a Letter of Attorney and your official ID card along with your applicant’s ID card.
